A 2010 study from Johns Hopkins University, published in Topics in Stroke Rehabilitation, demonstrated that early rehabilitation interventions were associated with shorter ICU stays and reduced overall hospital length of stay, reinforcing the clinical and economic value of early mobilization.
Stroke patients are most responsive to therapy during the first few weeks after the event. This period, often referred to as the “golden window” of recovery, is marked by heightened neuroplasticity—when the brain is most capable of reorganizing and forming new neural connections.
Delaying therapy may mean missing the period when the brain is most adaptable.
During early hospitalization, patients may receive:
1. Acute Phase (First 24–72 Hours)
2. Early Sub-Acute Phase (First Weeks to 3 Months)
3. Late Sub-Acute to Chronic Phase (Beyond 3 Months)
Although spontaneous recovery slows after three months, meaningful gains can continue for 12–18 months or longer with sustained therapy.

Rehabilitation should begin within 24–48 hours after stroke, once the patient is medically stable. The earlier therapy starts, the greater the likelihood of regaining lost skills.
